SLADE Membership
The current membership year is January 1, 2010 through December 31, 2010.

Levels of Membership
Active Membership—$25.00
- Must be a member of AADE (American Association of Diabetes Educators
- Any healthcare professional with an interest in diabetes, professional education, or research
- Active members, who must belong to AADE, have the right to vote, make nominations, and hold office at both the local and national levels.
Associate Membership—$35.00
- Any person with an interest in diabetes and/or any healthcare professional who is not a member of AADE.
- Associate members may serve on any committee.
To Join or Renew
To join SLADE or to renew your membership for 2010, please complete a membership application form and submit payment for membership dues. You may either complete and print a Membership Application form and send it with your check to the address on the form, or you may complete an online Membership Application form and pay immediately through PayPal.
