Please use one of the forms linked below to nominate a candidate for the 2010 Educator of the Year Award. The deadline for nominations is October 30, 2009.
Give brief descriptions of the candidate's performance as they relate to any or all of the following attributes/qualities of a diabetes educator.
- Contributes to diabetes education through patient care in an outstanding, unique fashion.
- Performs in a committed and dedicated manner.
- Uses innovative methods and ideas in his or her approach to planning the care of the patient(s).
- Consistently meets the physical, psychological, social and educational needs of patient and family.
- Uses the teaching/learning process in daily patient education and care of the person with diabetes.
- Effectively communicates with patients and families.
- Serves as a role model to the diabetes education community.
- Creates an atmosphere of mutual respect and trust with patients, families and fellow professionals.
- Establishes an environment where learning has proven effective.
You may include any additional information about the candidate which relates to his or her function as a diabetes educator.
